How Does Bayside Urgent Care Handle Workers’ Compensation Claims?
For all workers’ compensation claims, we provide patients with the DWC-25 (Status of Medical Injury form) completed at the time of service. We will fax this form to your Human Resources department or manager within 24 hours of your visit.
Our workers’ compensation services include but are not limited to:
- In-house x-ray imaging
- In-house prescription medication dispensing
- Drug screening
- Stitches for lacerations and cuts
- On-site sports medicine physicians
- On-site physical therapy
Our goal for our workers’ compensation patients is a full recovery after work-related injury and to minimize loss to the employer. If you experience an on-the-job injury, it’s essential to report the injury and have it evaluated immediately to receive compensation benefits. After your evaluation, we will recommend follow-up visits at Bayside Urgent Care Center or with specialists as well as any restrictions for recovery.
Does Bayside Urgent Care Accept Insurance?
We accept several major insurance carriers, as well as all workers’ compensation insurance plans. If you prefer to pay out-of-pocket, we also have flexible and affordable payment plans.
What to Bring When You Visit Bayside Urgent Care
To get the most out of our occupational health services and to help our medical team, be sure to bring:
- A list of your medications, vitamins, and supplements along with dosages
- Information about past surgeries and illnesses
- Dates of screening examinations and vaccinations
- Your primary care doctor’s address and phone number in case we need to transfer records
